Liability Waiver

Warning & Acknowledgment of Risks

I recognize and understand that martial arts training is a physical contact activity and that my participation may result in serious injury, including permanent disability or death, and severe social and economic loss. Risks may result from my own actions, the actions or negligence of others, the rules of participation, the conditions of the premises, or the equipment used. I recognize that other risks may exist that are not known or reasonably foreseeable at this time.

Assumption of Responsibility

I agree to inspect the facilities, equipment, and pairings prior to participation. I will immediately inform an instructor if I believe anything is unsafe or beyond my capability and will refuse to participate. I assume all risks and accept personal responsibility for any damages that may result from injury, disability, or death. I certify that I am in good physical condition and have no condition that would impair my participation.

Agreement to Participate

I enter martial arts training entirely of my own free will and understand the importance of following the rules of training and competition. I have been provided the rules and regulations of Yousef Brazilian Jiu-Jitsu Academy and agree to abide by them.

Medical Consent

In case of injury, I authorize Yousef Brazilian Jiu-Jitsu Academy staff, instructors, or responsible adults to provide or seek emergency medical treatment on my behalf. I assume all responsibility for the cost of medical treatment.

Release & Waiver

I release, waive, discharge, and covenant not to sue Yousef Brazilian Jiu-Jitsu Academy, its affiliated organizations, instructors, staff, participants, sponsors, landlords, or property owners from any and all liability for injury, death, or property damage sustained while participating, whether caused by negligence or otherwise.

Risk Agreement – Advisory of Rights & Responsibilities

All students are responsible for their own safety and the safety of others. Students have the right to excuse themselves from any activity they believe is unsafe. Students must train with respect, provide space for others, and report unsafe conditions or behaviors to instructors immediately.

Consent to Physical Contact

I understand that martial arts training involves physical contact with various parts of the body. Contact may occur between male and female students, adults and minors, or training partners. I agree to conduct myself appropriately at all times. If I feel uncomfortable, I may withdraw from the exercise and notify an instructor.

Social Media & Photo/Video Release

I grant permission to Yousef Brazilian Jiu-Jitsu Academy to take photographs and video recordings of me during classes, training sessions, and events. I authorize Yousef Brazilian Jiu-Jitsu Academy to use these images and recordings for promotional purposes, including but not limited to social media, websites, and marketing materials, without compensation. If I wish to revoke this consent, I must notify Yousef Brazilian Jiu-Jitsu Academy in writing.

Coverage for Minor Children

This waiver covers myself and any minor children I enroll at Yousef Brazilian Jiu-Jitsu Academy. As the parent or guardian of any minor participant, I agree not to bring claims against Yousef Brazilian Jiu-Jitsu Academy or its instructors, staff, students, or affiliates for injury sustained by my child, except in cases of criminal conduct. I agree to indemnify Yousef Brazilian Jiu-Jitsu Academy against any costs or fees arising from a breach of this agreement.

Arbitration Clause

Any disputes arising from participation at Yousef Brazilian Jiu-Jitsu Academy will be resolved by binding arbitration. If a suit is filed in court, I authorize the court to order the case into binding arbitration.

Severability & Durability

If any part of this waiver is found unenforceable, the remaining sections will remain valid. This agreement remains in effect indefinitely and applies retroactively to the beginning of training if signed at a later date.
